This screen you need to put in the Admin Username and Admin Password fields you existing admin account details (ie your root user) for MySQL.

Xibo will use those once to create the new database, and the user you specify under Database Username and Database Password.

The error message suggests that either those credentials were wrong, or that a firewall or other policy prevented access.
